Service Description

Our child psychiatry services are available for children from ages 5-17. All required forms must be completed before patient is booked. Referral Limitations: • Autism or autism spectrum disorder (ASD). • Learning Delays • Some Learning disabilities such as Dyslexia • Keywords for an ADHD assessment: Behavioural issues, lack at concentration, anger issues. • Substance abuse – past or present. Child Psychiatry Forms: • To be completed by the parent: o Weiss Symptom Record II (5 Pages) o Weiss Functional Impairment Rating Scale – Self Report (2 Pages) o Weiss Diagnostic Screen (4 Pages) o The SNAP – IV Teacher and Parent Rating Scale (2 Pages) • To be completed by the teacher o CADDRA Teacher Assessment Form (3 pages) o SNAP – IV 26 (1 Page) ​​ Additional Information: • We are fortunate to have psychiatrists that work on weekends and even through holidays. • We provide referral received and appointment booked notices. • We notify the patients of their appointment. • In our effort to keep low wait times, we do have a cancellation policy and a missed appointment fee if not cancelled prior to the 48hours of $100. • We do provide a courtesy confirmation call 72-48hrs prior, however it is ultimate the responsibility of the patient to confirm his/her appointment by noon of the day prior to the appointment.
